Labiaplasty

Relief, comfort, and confidence — guided by a surgeon published on the procedure.

Many women live for years with a discomfort they assume is simply normal — irritation in tight clothing, chafing on a run or a bike, self-consciousness in swimwear, or pain during intimacy. Very few realize it is both common and correctable. Labiaplasty is a straightforward outpatient procedure that relieves those symptoms by reducing enlarged or protruding labia minora, and for most women the relief is immediate and lasting.

What Is Labiaplasty?

Labiaplasty reduces and reshapes the labia minora — the inner folds of tissue — when they are enlarged or protrude beyond the labia majora. Some women are born with more prominent tissue; for others the change follows childbirth, weight change, or the natural process of aging.

The reasons women choose the procedure are usually as functional as they are aesthetic:

  • Discomfort or pinching in tight clothing, leggings, or swimwear
  • Chafing and irritation during exercise, cycling, or riding
  • Recurring hygiene difficulty
  • Dryness or soreness in the area
  • Pain or discomfort during intimacy
  • Self-consciousness that limits activities or clothing choices

Why Choose Dustin C. Derrick, MD

Dr. Derrick is a published co-author on labiaplasty surgical technique and classification. His work appears in Aesthetic Plastic Surgery, the peer-reviewed journal of the International Society of Aesthetic Plastic Surgery, in a paper that proposed a four-class system for describing labial protrusion and matched each class to an appropriate surgical approach.

Very few surgeons offering labiaplasty have contributed to the academic literature that shapes how the procedure is planned and performed. At Cosmetic Surgery of DFW, your procedure is guided by that same framework — the one Dr. Derrick helped define.

Understanding Labial Classification

The classification system Dr. Derrick co-authored describes labial protrusion in four classes. It gives surgeon and patient a shared language for exactly what is being addressed, and it guides which technique is most appropriate for you.

  • Class 1 — Within normal limits. The labia minora extend less than about 2 cm past the fourchette and remain within the labia majora. No reduction is indicated.
  • Class 2 — The labia minora extend more than about 2 cm and protrude beyond the labia majora. This is the most common reason women seek treatment.
  • Class 3 — Class 2 protrusion together with excess tissue above the clitoris — the clitoral hood, a separate area that is addressed at the same time.
  • Class 4 — The most extensive: Class 2 or Class 3 protrusion that also extends back toward the perineum, and in some cases the anus.

Surgical Approaches

Two established techniques are used to reduce the labia minora. The right choice depends on your classification, your anatomy, and what you want the result to feel and look like.

Direct Excision

Often described as the “clip and snip,” direct excision removes excess tissue along the outer margin. In the series Dr. Derrick co-authored, this approach was quicker to perform, preserved sensation along the labial edge, and did not produce visible scarring in the authors’ experience.

Wedge Resection with Flap Advancement

A wedge of tissue is removed and the remaining edges are advanced together, which preserves the natural free edge of the labia. It remains a valuable option and is well suited to certain anatomies.

Dr. Derrick will explain which approach he recommends for you, and why, at your consultation.

Your Consultation

Your consultation is private, unhurried, and entirely confidential. Dr. Derrick will listen to your concerns, examine the area, explain which classification best describes your anatomy, and recommend an approach. You will receive a clear, itemized estimate before you make any decision at all.

The Procedure

Labiaplasty is performed on an outpatient basis — you go home the same day. The procedure itself is short, and your anesthesia options are reviewed with you well in advance so there are no surprises on the day.

Recovery

Most women take a few days of rest before returning to desk work. Swelling and tenderness settle over the following weeks, and you will be given specific guidance on exercise, intimacy, and returning to normal activity. Complication rates in the published series were low, and the procedure is well tolerated.

Your Results

In the published series of more than 100 patients treated over ten years, most reported being pleased with their results. The goal is comfort and a natural appearance — not a dramatic change, and not something anyone else would notice.

A Patient's Experience

“This is the second time I’ve had the pleasure of having him as my surgeon, after a labiaplasty in 2025. I’ve never felt more personally cared for by a physician or his staff… He’s answered every question I’ve had and never rushed me and makes me feel as if I’m his only patient. I trust Dr. Derrick with any surgery without a second of hesitation.”

Labiaplasty is one part of a broader group of procedures addressing comfort, function, and appearance. If you are considering more than one concern, explore vaginal rejuvenation — which includes labia majora reduction, clitoral hood reduction, and labial fat grafting. These are frequently combined with labiaplasty in a single procedure.
