Notice to the wonderful patients of Dr. Dustin C. Derrick, MD:
After careful consideration, I am transitioning to a new practice, and as a result, I will no longer be with Fort Worth Plastic Surgery (FWPS) effective July 2, 2026.
I want to express my sincere gratitude for the trust you have placed in me as your surgeon and doctor. It has been a privilege to be part of your healthcare journey. I am committed to ensuring a smooth transition of your care and will be at my new office on July 2, 2026.
To help with this transition, fill out a medical release form. The form is to allow transfer of your medical records to the new office. Simply fill out and then either scan or snap a photograph of the completed form and email it to: [email protected]. If you were my patient prior to joining FWPS on 8/10/2021, my new practice will be the custodian of your records. If you became my patient on/after 8/10/2021, then FWPS will be the custodian of your records.
If you need follow-up appointments past July 1, 2026 please call 817-529-4314 to set up an appointment with my new practice. If you would like your care to remain with Fort Worth Plastic Surgery, please let the front desk know or call 817-810-0770. They have surgeons who can assume your care if you choose.
I want to assure you that your well-being remains my top priority, and I am dedicated to making this transition as seamless as possible for you.
